💰 The Business of Women’s Health Case Study - Did You Know?

Do you know what a RVU is and why it matters for women’s health?

Relative Value Units (RVUs) are a system used to calculate physician payments in the United States. Implemented by the government (CMS) and the American Medical Association in order to attempt to keep costs down, they are based on a number of factors including:

  • Physician work: Includes the time, skill, and mental effort required to perform a service, as well as the physician's expertise

  • Practice expenses: Includes the expenses of the practice

  • Professional liability insurance: Includes the cost of professional liability insurance 

But, they cause double discrimination in women’s gyn surgery and significant downstream impacts on women’s health outcomes.

For example, gynecologic surgeons get paid less for the same procedure on women vs. men, and therefore they earn less overall. And because healthcare is a business, the care that makes money often gets prioritized.

In addition, fewer codes for gynecological surgery vs other surgical fields make it harder to bill for the actual work done. We have just seen major progress here in one speciality as Endometriosis codes expanded from 9 codes to over 100 in 2023. But we still have a lot of work to do.

Why is this?

RVU’s are set by the RUC - the American Medical Association (AMA)/Specialty Society Relative Value Scale Update Committee. The RUC is a group of physicians who advise the federal government on the resources needed to provide medical services. 

The RUC's recommendations to the Centers for Medicare & Medicaid Services (CMS) affect the relative values assigned to physician services, which in turn affects how much physicians are paid. The RUC's work includes: 

  • Making recommendations for new and revised codes 

  • Updating relative value units (RVUs) annually to reflect changes in medical practice 

  • Submitting recommendations to the CMS that improve the data used to create RVUs 

The RUC was established in 1991 by the AMA and medical specialist groups. 

Challenge is … on the RUC, OBGYN + Gynecologic surgery have one combined vote.

But the real challenge is there is a legal requirement for neutrality in billing.

In order to provide more RVU’s to OBGYN or women’s health, they would have to be taken from somewhere else — and another speciality would have to agree to give them up.

Right now, it’s a no-win system for women’s health.

The downstream impacts of this include:

  • Hospitals reduce access to surgical operating rooms for gyn surgery because they aren’t profitable

  • Gyn surgery is going cash-pay — limiting care access to women both financially and geographically

  • Fewer codes mean it’s harder to accurately bill for procedures, which then in turn, means the subspeciality loses money.

  • Surgeons hesitate to go into gyn surgery because of the pay discrepancies, which then creates a supply-demand issue with provider shortage.
