Last week, we had an insightful session as a part of the Womenā€™s Health Mini-MBA with Hope Yates, Chief Strategy Officer for Womenā€™s Health at Columbia University, discussing the challenges and opportunities in womenā€™s health.

Here are 7 key takeaways:

1ļøāƒ£ Womenā€™s Health is Underfunded: Womenā€™s health services are consistently underfunded. For example, gynecological procedures often receive significantly lower reimbursement rates than male-specific procedures. In fact, male-specific procedures have a median RVU of 25.2, compared to just 7.5 for female-specific procedures, according to a 2015 study. A subsequent 2017 study has shown some improvement in specific areas.

2ļøāƒ£ Impact of Budget Neutrality: The Centers for Medicare & Medicaid Services (CMS) are required to maintain budget neutrality. This restricts RVU increases for womenā€™s health without cutting reimbursement elsewhere, limiting progress. As of 2024, the CMS conversion factor has dropped to $32.74, further straining the financial viability of womenā€™s health services.

3ļøāƒ£ Disparities in Reimbursement: A 2017 study found that 72% of male-specific procedures were weighted higher for work and total RVUs compared to similar gynecological procedures, with 84% being compensated at a higher rate. For instance, a prostate biopsy is worth 4.61 RVUs, while an endometrial biopsy is only worth 1.3 RVUs, despite similar complexity.

4ļøāƒ£ Columbiaā€™s Mental Health Integration: Columbia University has embedded mental health services into its womenā€™s health department, addressing conditions such as postpartum depression and anxiety. This model has grown significantly, with over 13 mental health providers now offering virtual care, improving access and generating additional revenue. Despite these efforts, Columbia still loses over $3 million annually on womenā€™s health services due to insufficient reimbursements. However, the integration of mental health helps improve both outcomes and financial viability.

5ļøāƒ£ Solutions for the Future: Advocating for changes in reimbursement models and increasing representation of womenā€™s health professionals in decision-making is essential. Currently, women represent over 84% of OB-GYN residents but hold only one seat on the 31-member AMA RUC committee, which controls RVU recommendations.

6ļøāƒ£ The Need for Innovation: Telemedicine and AI are key to improving womenā€™s health care. Telehealth adoption increased by 38X during the pandemic and continues to grow, with 76% of U.S. hospitals now offering telemedicine services. Columbiaā€™s integration of virtual mental health care has cut operational costs while maintaining high levels of patient engagement using telemedicine without expensive real estate costs to provide care.

7ļøāƒ£ Creating Opportunities in Womenā€™s Health: Hope shared how she created her own role at Columbia by identifying unmet needs in women's health and leveraging her experience from public health and the private sector. Her advice for those looking to enter the women's health space? Say yes to new opportunities, take risks, and be open to building something new. Passion for solving real-world issues and perseverance are key to making an impact in the field.
